Who ever did should lose their job. Well, not sure that I would be so quick to call for someone's job, but I'd certainly look at who signed your occupancy certificate and start there. Either BS dropped the ball and issued op without the certificate or else someone supplied a false certificate. If you had an independent inspector to inspect your home prior to handover this would have been picked up. For a decade now I have been finding poorly installed or missing ceiling insulation in new and established home. Just because there is a certificate it does not mean insulation is properly fitted. Definitely worth paying an independent inspector to go through the property with a fine-toothed comb - Even if the builder tells you that it will slow things down, which is clearly to avoid being held responsible for their stuff-ups.
