In two ensuites I have the toilet and bathroom together. For the common bathroom they are separate and the master ensuite they are separate. When designing my house I made sure that both stories had a separate bathroom that guests could use. Depends on space I guess. Space ( or lack of it ) is the main consideration. If you have a large house you can afford the extra room for a separate toilet. If you live in a small house it may not be the best option or best use of the space available.
