Cannot align shower arm

My shower has been re-tiled. I now cannot properly align the shower arm. Upright is too loose. Half a turn more and it is firm but upside down. 8 o'clock is fully tight. There is minimal thread protruding. It came with its own matching backing plate.

I'll look into different shower heads and ask the plumber about some engineering and see what he says. Thanks
