I like the idea of this but in speaking with some builders a few want to drop the outside area 1 or 2 courses. Is there any major concerns with them being the same height? I did. 1 or 2 courses is approx 86mm or 172mm. The measurements are moot really though. Think along the lines of it either drops down when you step out or it doesn't. Yes, if you have floating floors inside then tiles on a sand and cement base on the alfresco outside. The outside area will be something like 30mm higher. A bit hard to waterproof.
