How much of a problem is this? Does anyone know if it can be fixed?

I suggest they should remove the dynabolt, clean away the spall, put some kind of priming agent on piece of slab to be repaired and repair it with a high strength quick curing repair mortar. They should leave it a set amount of time and redrill the hole for the dynabolt and put the bolt back in.
