For me personally, I don't like toilets in bathrooms (ensuites I'm okay with, just), but not in main bathrooms. If you have guests staying, if someone is using the shower then the other guests can't access the toilet. And people always need to use the toilet when someone is using the shower or the bathroom for other reasons. That would then mean that you would have to be happy about people accessing the toilet in your ensuite, not sure how you feel about it. I think Jo's idea is great, gives you the best of both worlds without sacrificing too much. Looking at some of the designs on websites, they are all selling very similar products.
