Tell them you want to speak to their boss ...get names etc etc etc... then let them tell u the reasons why etc. etc... regurgitate and confirm (ie paraphase what they said) and get their agreement. Note the time and place/who ... then at the very end tell him nicely that it is unfortunate that you did not get the outcome you want and that you will lodge a formal complaints now that you have all of his details via the banks' complaint process. By law they have to respond to you within 48hrs (i think) and if you don't get a reasonable response tell them you will file a complaint with the banking ombudsmen. They will be petrafied...their bonuses and performance measures are all based on complaints and customer resolution as well. I got my issue resolved within 24hrs as soon as i threatened to file a complaint. and its your RIGHT to file if you are not happy.
