If you go through Lincoln Sentry it's the centre hinge on a finista BF-05 bifold door system. I'm not sure if you can buy them individually or have to get the whole set. Yours are from Hettich and should be part number 1077338 that's the plastic part. The metal bit on the other side is part number 1077339. They may be old part numbers but it should get you close.
