It really depends how much work you're willing to do. A caroma concorde pan with a slimline or uniset cistern will likely be the easiest to install but be sure to take measurements of the cistern and also the pan setout (distance from the wall to the centre of the waste). Take these measurements along with a few photos to Tradelink or Reece plumbing and they will be able to show you a few different options. It does seem to be a slimline cistern. The link piece appears to be short/not close enough to the pan resulting to that gap which has some kind of warped plastic in it. Is there a way of replacing this set up with a close coupled one? Close coupled seem to be more aesthetically appealing. I dare say you will have to move the waste closer to the wall to suit a C/C and then re-tile... There a p trap base and come with adaptor to convert pack to a s trap. A normal set out toilet like yours is set of the wall standard 180 to 240 mm centre where close couple need set off wall 140 think it was.
