However, one builder mentioned that the waste pipe from the new basin cannot be put undergroud because it would need to dig into the concrete block base that might raise structure concern. In other words, the pipe will be seen on the ground when the job is done. Is that true? Not recommended. If the new vanity is against an outside wall he will place the waste pipe out through the wall, where it will turn down underground immediately. That is very common. I believe sewage waste pipes can't be routinely laid on-ground, they must be protected somehow. Anyone know about that? If the new vanity is not against an outside wall there will indeed be a difficulty. We have two such in upstairs bathrooms, and under one in the kitchen the waste pipe is hidden by a false ceiling on its way outside, and under the other in the laundry it is visible, running across just under the ceiling. Can I ask the reason why it is not recommended to cut a trench into the concrete slab inside the bathroom? After construction, it would be two small bathrooms in my single-story house. That's why I worry whether it would look ugly/unconvenience if the pipe sits on the ground even though it is protected. In a slab bearing the loads of the roof, brick walls, and any upper stories, stesses tend to deflect it especially near to weight bearing walls. Trenching will compromise that. Also in your slab will be reinforcing steel which requires a covering of concrete to protect it against corrosion. Trenching will expose this, and it may have to be cut. In a forum like this one I could not say anything but "not recommended". What about an unobtrusive box-section ledge running along the wall-floor junction, hiding the waste pipe on the way to the nearest wall?
