You put in a smaller height floor joist to that section and then pack it up on site so that the finished level is the same as the remaining floors once bedded by the tiler. It is quite labour intensive and thus costly from a Builder's perspective. I'll look into different shower heads and ask the plumber about some engineering and see what he says.
