Vehicle crossover form

About to start Building a new home in South west Sydney.

My builder has asked me to submit a vehicle crossover form with council which will be used for CDC approval.

Form requires license and Insurance details of a Concreter.

Has anyone done this in the past and can recommend a concreter who should be able to help with this ?

When we did our crossover we asked for a heavy duty concrete, so that the trucks carrying, e.g., gyprock wouldn't crack the concrete. Maybe worth thinking about. From memory it was a minor price difference.
